By attending the CANA Program, hereinafter referred to as "ACTIVITY", you, for yourself, your child(ren), your partner, your legal representatives, heirs and assigns, hereby waive, release, and discharge the meeting venue, the California Association of Nurse Anesthesiology (CANA), HollandParlette, and any of their affiliates, directors, officers, employees, agents, contractors, or volunteers from any and all liability for any and all losses or damages resulting from bodily injury, illness, permanent disability, and/or death resulting directly or indirectly from exposure to or infection with COVID-19 (or associated variants) before, during, or after participating in the ACTIVITY. Safety Requirements & Recommendations: By checking this box, you agree to follow the mask and social distancing mandates and any other recommendations/safety requirements at the time of the event, as directed by state and local health authorities (including the city, county health authorities in which the venue is located), the venue, or by CANA, regardless of vaccination status. This may also include providing proof of vaccination or a negative COVID test. COVID-19 Exposure or Other Illness: By checking this box, you agree that if you have been diagnosed with COVID-19 or have experienced any of the symptoms within 48 hours of the event OR you generally feel ill, you will cancel your registration and will not attend the meeting. Also by checking this box, you agree that if you have been in close contact with an individual who has been diagnosed with COVID-19, within 72 hours of the event, that you will follow the CDC guidelines to determine whether you should attend.
